They are almost all the result of one person being forced to pay another person's bills. I would not expect the HHS to discover that.
The major factor in driving up costs is the government adding trillions of taxpayer $ to the system, just as in education and housing.
Problem in housing is state/local regs restricting development, not subsidies. Cmon man.

Here is some data on what health insurance cost in 1960, before the big social medical care programs. from the 1960 today website. Notice that health insurance for an entire household was less than $10 a month--the equivalent of $741 a year in today's money.
